As you'd expect from a next-generation wellness-focused plan, our Preventive Health Actions aren't meaningless promises to head to the gym, but consequential acts:
- 1. Fill-out a personal health questionnaire
- 2. Complete an age and gender-specific health screening
- 3. Participate in a biometric screening
That's all it takes for members to qualify for SeeChange Health's financial incentives. And for those members with specific chronic conditions, there are additional Chronic Condition Health Actions to help them deal with them—and be rewarded for doing so.
